Identification and implementation of the best reorganisation strategies aimed at the emergency-urgency network of the ASST dei Sette Laghi and the emergency-urgency pathway of the Macchi hospital

Technical support to ASST Sette Laghi for reorganising the network of emergency departments in the area and improving the emergency-urgency processes of the Macchi Hospital in Varese. The project included an in-depth analysis of flows, logistical and organisational issues, and the construction of improved scenarios, through the development of a dynamic simulation model, to test the impact of different solutions and guide reorganisation choices.

Business Challenge

Long waits for patients for initial visits
Extended stays of patients awaiting hospitalization (boarding)
High operational pressure on staff
Need to objectively and predictively evaluate alternative scenarios

Milestones

1. In-depth analysis of historical data related to emergency room visits, length of stay, priority codes, outcomes, and hospitalisation times.

2. Mapping of current pathways and identification of the main organizational and structural bottlenecks.

3. Construction of the patient flow simulation model within the emergency department, configuring the different phases of the process and operational constraints.

4. What-if analyses conducted through the simulation model, to test various reorganization solutions and assess their impact on key indicators (waiting times, area occupancy, patient turnover, resource utilisation).

5. Definition of implementable solutions, consistent with structural constraints, available resources, and improvement objectives.

Conclusions

The project allowed ASST Sette Laghi to have an analytical and predictive basis for the reorganization of the Emergency Department of Varese, addressing in a structured manner the critical issues arising from the prolonged stay of patients awaiting hospitalization.
Thanks to the simulation model, it was possible to anticipate the effects of choices, objectively compare scenarios, and guide the organizational transformation on solid and measurable bases, with the aim of improving accessibility, quality, and sustainability of the emergency-urgency pathway.
